5/4/2023 EnchantaVaughn Approved (AM) Upon arrival, client was in a joyful mood as evidenced by his smile and genuine spirit. Client presented himself with neat and clean attire and a fresh haircut. Client displayed appropriate social behaviors in the classroom setting, being a big helper to all of his peers. Client was very well mannered transitioning into all activities and showed a lot of progress with objectives 1, 2, and 3. (PM) Client presented himself in a cheerful, pleasant, and respectful manner throughout the day. Client was able to stay focused during class assignments, interacting well with his peers. Client was polite and displayed appropriate behaviors and appeared to be achieving objective 2 and 3, as evidenced by him listening and following directions while showing self control and confidence. (EOD) Client was compliant to positive reinforcement staying attentive to his assigned work and dismissing from class in a positive mood, ending the day meeting all of his objectives, as evidenced by his cooperation throughout the day. (AM) QMHP welcomed client back to school and provided support and encouragement to ensure a good start to his day and to be on track with achieving his goals and objectives. QMHP monitored client individualized behaviors in collaboration with his teacher and other mental health professional. (PM) QMHP provided praise when the client continued to display positive behaviors throughout the day, remaining on task and was very helpful to his peers. (EOD) QMHP explored client feelings about different subjects in school and how to manage his emotions and self esteem. QMHP encouraged teacher to involve client in cooperative learning groups and to promote participation in social activities. (AM) Client greeted QMHP and said good morning. Client verbalized positive feelings about the first day of school and was excited to meet his new teacher. Client demonstrated understanding of goals and objectives. Client displayed high level of participation in class, lunch, and gym settings. (PM) Client responded in a positive manner acknowledging the positive reinforcement that the QMHP provided by saying "thank you" while presenting a smile. (EOD) Client expressed that his favorite subject in school is math and his least favorite subject is writing. Client exhibits moderate self esteem when it comes to critical thinking, often hesitating and saying "I don't know" before answering a question. Client was involved in effective learning group, increasing interest and ability to work independently.
